Basic Care
I acknowledge, understand, and agree to provide the adopted dog with proper food and fresh water at all times, a safe and clean sleeping environment, and a yard with a fence sufficient in height to keep the dog under my care, customer, and control at all times, or leashed access to an outdoor area (if living in an apartment, condominium, or other residence without a yard). I will provide the adopted dog with sufficient means of humane exercise such as walks, hikes, or visits to parks or other dog-safe environments.
Veterinary Care
I acknowledge, understand, and agree to provide all veterinary treatment as necessary to protect the adopted dog's health, including but not limited to: booster shot(s) for DHPP, Rabies vaccine(s) as required by Arizona state law, and Bordetella (Kennel Cough) vaccine(s), as appropriate.
